There are a few options that I can recommend. If your child has worked on TV shows and/or Film, the best option is to select two to four strong scenes for the reel. It is ideal to show a range of comedy and drama. Now this is assuming that your child has enough footage from previous jobs. Also, it is great to have well known actors in the scenes but make sure you child not completely over shadowed by the other actor.
If your child is just starting out, another option is to pay to have a video professionally shot and edited. There are many companies out there that will offer to film scenes for you. Investigate carefully. Don’t pay an arm and a leg for this.
For kids, I actually recommend Breakdown Services (actors access). They can film your child just doing a slate with a brief introduction. Great way to show off their personality!

You can easily do this on your own. We use a flip camera that cost about 125 and we record all of our kids’ auditions, monologues, dance routines, etc. and we put some of them on youtube and her agent sends people there. Make sure your kid doesn’t look like they are making a clip for match.com some I’ve seen are really sad and not in a good light.
