That is a great question. If your daughter is a member of the Screen Actor’s Guild, I suggest you list her SAG member number on the sign in sheet. I would not list the social security number on the sign in sheet since casting just throws those sign in sheets out when they are done using them and you don’t need anyone getting their hands on your personal info. What’s great is now that most offices use the electronic systems, they have your kid’s records in their systems so you don’t have to worry so much about listing it all on paper.
Lastly, I’ve never heard of anyone not getting hired because they missed something on the sign in sheet. As long as they have the name and contact info, it’ll all work out just fine.
